Intro A E F#m D (I'm not quite sure what variation of these chords he uses in the intro and outro, but you can give it a shot by watching this video: https://www.youtube.com/watch?v=QO2tIOrZVa4&fmt=18 )
Verse (mostly power/barre chords) F#m C#m/D C#m D A E x2 (root on 7th fret, 5th string for the E chord)
(the C#m/D should be played like this: D|-7-| pinky finger, with ring finger ready on 6th fret A|-4-| index finger E|----| Then just lift up pinky finger to make C#m with index and ring finger)
Interlude in the middle of first verse: F#m, then lift up index finger to play open E on the 6th string
Chorus A E F#m D (repeat as necessary)
(for the "I hope your faith..." part, play F#m E D; if you wanna get really specific, the E is played with the root note on the 7th fret of the 5th string)
Bridge A Bm C#m D x3 then E
Outro A E F#m D

Bridge/Outro C G Am GF (play open 6th string E when necessary on drawn out F chord) x2
On the word "been" in the "There's a fondness, but I can't remember where, just where I've been..." part, play G, then Am on the word "pen", GF (at "...into my skin..."), G ("Baby, here's where we begin")
End with C G Am GF x3 then G, end on C
