(root on 7th fret, 5th string for the E chord)

(the C#m/D should be played like this:
D|-7-| pinky finger, with ring finger ready on 6th fret
A|-4-| index finger
E|----|
Then just lift up pinky finger to make C#m with index and ring finger)

Interlude in the middle of first verse:
F#m, then lift up index finger to play open E on the 6th string

Chorus
A E F#m D (repeat as necessary)

(for the "I hope your faith..." part, play F#m E D; if you wanna get really specific, the E is played with the root note on the 7th fret of the 5th string)
